Remote Advance Care Planning Facilitator
We’re seeking a compassionate and skilled Remote Advance Care Planning Facilitator to conduct meaningful ACP conversations with patients via phone or video. This is a remote, part-time role perfect for someone with healthcare experience who wants to make a tangible impact on patients’ lives without the constraints of traditional clinical practice.

Key Responsibilities
Patient Conversations (Primary Focus)
Conduct structured ACP conversations with Medicare patients via phone or video (20-40 minutes per conversation)
Use Affirm Health’s conversation protocols and platform to guide discussions about healthcare values, goals, and preferences
Help patients think through important questions: What matters most to you? Who should make decisions if you can’t? What level of medical intervention aligns with your values?
Adapt your approach based on patient health literacy, emotional state, and complexity of situation
Recognize when cases require clinical expertise and escalate appropriately
Documentation & Workflow
Document patient preferences using the Affirm Health ACP platform (structured templates and forms)
Complete advance directive paperwork with patients during or immediately after conversations
Ensure all documentation flows seamlessly into the practice’s EHR system
Maintain detailed notes on conversation outcomes and any needed follow-up
Quality & Improvement
Participate in regular case reviews and coaching sessions
Provide feedback on conversation protocols and platform functionality
Share insights on what’s working and where patients struggle
Help refine the balance between AI and human facilitation
Required Qualifications
Experience:
2+ years in healthcare setting (hospice, palliative care, care management, patient navigation, or similar)
Previous experience with advance care planning conversations strongly preferred
Familiarity with end-of-life care and patient decision-making
Experience with Electronic Health Records (athenahealth or Epic experience a plus)
Skills & Attributes:
Exceptional ability to have difficult conversations with empathy and professionalism
Active listening skills—you genuinely care about understanding what patients value
High emotional intelligence and cultural sensitivity
Comfortable with ambiguity and complex family dynamics
Strong organizational skills and attention to detail
Tech-savvy: comfortable learning new software platforms quickly
Self-directed: can work independently while staying connected to the team
Important Note: This is a facilitation role, not a clinical position. You’ll be guiding conversations and completing forms based on established protocols—not providing medical advice or making clinical recommendations. We provide comprehensive training on our conversation methodology and clear escalation criteria.

Work Environment & Compensation
Location: Fully remote (work from anywhere in the US)
Schedule: Flexible, part-time to full-time (15-40 hours per week)
Most conversations scheduled during business hours (patient preference)
Some evening or weekend availability helpful but not required
You’ll work with our scheduling team to build a workload that fits your availability
